• Provide services in collaboration with all circles of support; Case Management team, School Counselor, Mental Health Providers, etc. – to obtain optimal functioning level within the community.
• Provide engagement, bridging, & transition support to clients/families.
• Provide support to clients centered around coping skills, hygiene, daily living activities, self-care, safety plan, and socialization.
• Help identify non-healthcare needs (ex. Housing, employment, social support) that may be impacting the client/family.
• Timely and accurately document the delivery of services in an electronic health record.
• Develop a plan of care (POC) for each client that includes clearly defined and appropriate referrals and access to health providers to address health needs.
• Meet with each client to review the POC every three months during the service period and conduct case conferences every two months; monitor the client’s progress in achieving goals detailed in the POC.
• Advocate for client/family needs with other professionals including Case Managers, Psychotherapists, etc.
• Attend all required training.
• Participate in multidisciplinary team meetings for quality delivery of service.
• Other duties as assigned by the Program Manager.
Required Qualifications and Experience at This Level:
• Bachelor's Degree in a related field.
• Must be culturally competent.
• Refugee resettlement program experience a plus.
• Solid organizational and follow-up skills.
• Computer literacy – learn and adapt to different IT platforms.
• Must be fluent in English.
• Being bilingual in at least one other language such as, Farsi, Dari, Pashto, French, Swahili, Spanish or any other foreign languages, is required.
• High degree of independent judgment, flexibility, and problem-solving skills.
• Must have a valid driver’s license, automobile liability insurance, and knowledge of the city.
• Some evenings or weekends may be required.
